What should I bring to a medical card evaluation?

A government-issued photo ID that shows your North Dakota or Minnesota address, and medical records that document your qualifying condition. Please get your records to us before the visit so Dr. Ewals has time to review them. Not sure how to request records? Call and Will can walk you through it.

Does an evaluation guarantee I'll get a medical card?

No. Dr. Ewals decides whether certification is appropriate after reviewing your records and talking with you. If she certifies you, the state still reviews your application and issues the card.

How long does a medical card last?

Each state sets its own rules for how long a certification and registry card stay valid, and those rules change. We'll confirm the current period for your state at your visit.

Do I need lab work before a men's health consultation?

Usually, yes. Call us for a lab order, complete the labs, then book your visit so Dr. Ewals can review the results with you. Results are generally confirmed with early-morning blood tests, not symptoms alone.

Can I send my medical records by email?

Please don't. When we call you, we'll explain how to send records securely, for example by fax to (701) 757-0974.

Do you take insurance?

No. We don't bill insurance for these visits. Keep your receipt; if you have questions about reimbursement or taxes, ask your plan or a licensed tax professional.

What if I need to reschedule?

There's no charge to reschedule. Fees are not refundable, so please call as early as you can if your plans change.

Do you offer telehealth visits?

Some visits may be available by video or phone, depending on your state and whether you're a new or returning patient. Ask when you schedule and we'll tell you what applies to you.
